We here at Del Monte Assisted Living and Memory Care are pleased to share that we are one of the largest assisted living facilities in the Northern California area to participate in the Assisted Living Waiver Program. Licensed for 158 residents and offering memory care, we are in the city of Stockton in San Joaquin County ( Map Link ). We are centrally located between the I-5 and Highway 99 for easier access to our facility for families to come and visit their loved ones.

If you’re currently working with a patient that is already approved for enrollment with the Assisted Living Waiver Program, we are happy to review their referral for fast-track admission to our beautiful facility. 

Do you have a resident who is pending approval? No problem, we can still assess them and if they are a great match for us, we can put them on our waiting list for moving into our facility.

Do you have a resident who has Medi-Cal ( Click Here ) and needs assistance with applying for the Assisted Living Waiver Program? We can help you connect with a trusted Care Coordination Agency to get your patient onto the waitlist and complete the application.
